A check for $25 dollars must accompany the form.
Each paid entry will be allowed one entry in each category.

We will assume your space needs will fit into a 10x10 approximated pop-up sized space. If you wish additional room please indicate your needs when you send in your entry.

WAIVER of LIABILITY: In consideration of your acceptance of this entry form and fee, I, the undersigned intending to be legally bound, do hereby for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against the Silver Lake Sand Dunes Area Chamber of Commerce, KCBS, and their agents, successors and assigns for any and all injuries suffered to me in this event. I further grant permission to the Silver Lake Sand Dunes Area Chamber of Commerce and/or agents authorized by them to use any photographs, videotapes or any other record of this event for any legitimate purpose.
